Travel Insurance

Please complete this insurance form if you are purchasing travel insurance.  Please only send in one form per traveler (ie:  multiple family members should each complete a form).

November Joint Meeting Recap:

Greetings Carnegie families!
 
We had an informative meeting last night for our Band and Orchestra families.  Here are the presentation slides from last night and below are the forms handed out last night.
 
MBUSD Required Forms due by December 1, 2015
Please return all 3 forms [stapled together with your student’s name clearly indicated] to the Orchestra Room.

1. Student Field Trip Code of Conduct
2. Parent Waiver and Release for the Use of Student Images and Student Work
3. Parent Parent Permission for Student Participation in Off-Campus-Carnegie Hall*

Leave page 2 requiring teacher signatures blank. We will hold onto those forms and pass back out to the students to obtain teacher signatures in the month of May, 2016

AIRFARE INCREASE
Please note that our airfare per person has increased from $350 to $600, bringing the total performer cost from $1,800 to $2,050.  We have done extensive research to try to mitigate the original estimated airfare of $350/pp.  We are traveling on a holiday weekend with a direct non-stop flight from LAX to NYC and $600 is the best we are able to do for booking 318 [students/chaperones/staff] people – on multiple flights with multiple airlines all flights departing from LAX the morning of 05-26-15 and all flights arriving late afternoon/early evening of the same day.  The $250 increase will be added to the March 2016 installment, and we have been guaranteed by our travel broker that the cost ceiling is $600/pp.  Please note the following language that we have been putting out since the original flyer for the Carnegie trip.
 
"NOTE: Airfare is quoted based on current industry standard and will be confirmed 6 months prior to departure.  A price adjustment +/- will be made at that time. Items not included in above rate: Transportation and rental of instruments, checked bag fee, 3 lunches.  Every effort will be made to fundraise for the transportation and rental of instruments and checked bag fees.  If Boosters does not reach its target fundraising goal then those items will be added to the Performer Package."

We need Everyone’s help with Carnegie Fundraising and we thank you ahead of time for your support for our 265 Band and Orchestra students. There are many ways you can help. Select what you're mist comfortable with. The important thing is your participation. We can work together to raise the money needed for instruments, financial assistance and luggage fees. September Carnegie Chaperone News

Thank you to the many parents who came to the meeting.  We appreciate the time you have set aside to learn about roles, responsibilities, and requirements related to chaperoning the 260+ student musicians on the Carnegie trip. If you were not able to make it to the meeting, please review the documents below. Those who attended may find information below a good refresher.

In June, we asked for parent interest in chaperoning. This month, after understanding roles, responsibilities, and requirements, we ask for your commitment.

About your Final Installment
A note from our VP of Finance:​

Greetings Orchestra Families!
I'd like to share some details about the "final installment payment" for your student's trip to Carnegie Hall. As you are aware, the cost per student staying in quad accommodations (4 students per room) is $2050. This DOES NOT include the cost of checked baggage. If we are unable to raise enough money to cover that cost, we will be collecting the $50 checked baggage fee from every performer on April 20.
I'm preparing a final statement for each participant on the trip and will begin sending them to families by email next week. That statement will list every payment that I've recorded for your student(s), along with the credits your student(s) have earned (from scrip/candy sales). It should be clear how I arrived at your final balance due. If you have any questions, please contact me ASAP.
Scrip and candy sales to earn credits for your student's trip end on March 10. Families participating in these fundraisers will receive their statements on or about March 13.

The final payment  is due NO LATER THAN March 18th.  Please, no exceptions.

Due to the unique nature of everyone’s final payment amount, we encourage everyone to pay by check. For those families that have been using the online payment option, the installment payment option ($225 + fees) will continue to be available and we’ve added a $500 + fees option.  

Chaperones:  I will send your statements and your student's statement next week as well. Your payment is based on your room choice and whether or not you have someone else staying in your room.

Fundraising

We are joining with the Band Boosters to raise the necessary funds not covered by our contract with Artist Travel Consultants. (See original information flyer sent home in June.) The biggest expense is related to the rental and transportation of the necessary instruments and financial assistance.
